A New Case Manager from Family Health Centers of San Diego Shares Her Experience

For several years, ISN has partnered with Family Health Centers of San Diego (FHCSD) to provide the Rotational Shelter guests with great case management. This year, FHCSD covered the San Diego Beaches region with a seasoned case manager who was new to ISN's program. Iliana Q. from FHCSD did an amazing job and shares her experience as a first-time Rotational Shelter Case Manager.

My first year with ISN was an incredibly rewarding and meaningful experience. As a Case Manager II, I have had the privilege of working closely with individuals and families experiencing homelessness, helping them navigate barriers related to healthcare,  and other essential services. Through these interactions, I have witnessed firsthand the resilience and determination of our clients as they work toward stability despite significant challenges. Building trusting relationships with clients has been one of the most valuable aspects of my role, allowing me to provide support, encouragement, and advocacy during some of the most difficult times in their lives. 


In addition to working with clients, I have had the opportunity to collaborate with church coordinators, volunteers, and community partners who consistently demonstrate compassion and dedication to serving others. Their willingness to step in and provide resources, encouragement, and practical support has made a significant impact on the lives of many of the individuals and families we serve. These partnerships have contributed to numerous success stories, including families transitioning from homelessness into permanent housing, clients receiving essential furnishings and household items to make their new homes comfortable, and individuals successfully connecting to healthcare, employment, and supportive services. Seeing clients achieve goals that once seemed out of reach has been both inspiring and motivating. These experiences have reinforced my passion for case management and strengthened my commitment to helping individuals and families move toward long-term stability and self-sufficiency.
